UPDATED FALL SPORTS MEETING SCHEDULE, PICTURE DAY, AND REQUIREMENTS TO PARTICIPATE

Attention #cavaliernation:  The 2017-2018 school year is fast approaching and the OHSAA start date of August 1st to open the Fall Sports Season is just around the corner.  For any and all student-athletes please ensure you have proper paperwork is complete to participate as a Cavalier student-athlete.  Below is a list of items you must complete as well as a link to necessary forms. 
 
The following timeline will be followed to open the Cavalier Athletic Season for all Fall Sports:
 
  • July 30:   All Coaches (Fall, Winter and Spring) Meeting  5:00-6:30pm Cox Gym (Dinner provided by Dennis Murphy)
  • August 1-3:  Tryouts
  • August 3:  Uniform Distribution and Roster Completion
  • August 4:  Picture Day and Cavalier Creed Filming

Downtown Photography will be here on Friday, August 4th for individual and team pictures and the following schedule will be followed for this date:

  • Lady Cavalier Tennis 9:00 am OUC
  • Lady Cavalier Volleyball 9:30 Hatton Gym
  • Lady Cavalier Soccer 10:45 am Hern. Field
  • Cavalier Cheerleading Noon Hern. Field

  • Camera 2:
  • Cavalier March Band 9:00 am Hern. Field
  • Cavalier Cross Country 10:00 am Hern. Field
  • Cavalier Golf 10:15 am Hern. Field
  • Cavalier Boys Soccer 10:45 am Hern. Field
  • Cavalier Football 11:30 am Hern. Field
Fall Sports Meet the Team is Wednesday, August 16th at the "O" beginning at 5:30pm and this will double as our required OHSAA Parent Night so one parent/guardian must attend with every Fall Sports Student-Athlete.

I.                  REQUIREMENTS FOR PARTICIPATION

 

 

 

You are NOT eligible to try out for or compete in any sport until the following are completed:

 

 

 
  1. PHYSICAL EXAMINATION FORM completed and the form turned in to the Athletic Department.

  2. ALL ACADEMIC ELIGIBILITY REQUIREMENTS are fulfilled.

  3. EMERGENCY MEDICAL AUTHORIZATION FORM is completed and turned in to the Athletic Department.

  4. STUDENT HANDBOOK ACKNOWLEDGMENT FORM is signed by both student and parent/guardian and turned in to the Athletic Department.

  5. RESPONSIBILITIES OF PARTICIPANTS IN ATHLETICS FORM must be signed by both student and parent/guardian and turned in to the Athletic Department.

  6. PROOF OF INSURANCE must be signed by both student and parent / guardian and turned in to the Athletic Department.

  7. HIPAA FORM must be signed by both student and parent/guardian and turned into the Athletic Department.

  8. ASSUMPTION OF RISK AND RELEASE OF ALL CLAIMS is signed by both student and parent/guardian and turned in to the Athletic Department.

  9. STUDENT-ATHLETE SOCIAL MEDIA AGREEMENT AND POLICY FORM is signed by both student and parent/guardian and turned in to the Athletic Department.

  10. CONSENT TO PERFORM URINALYSIS FOR DRUG TESTING is signed by both student and parent/guardian agreeing and consenting to drug testing and turned in to the Athletic Department.

  11. TRANSPORTATION AND ATHLETIC FEE paid to the Athletic Department or satisfactory arrangements for payment are made by the first official contest.

  12. SUDDEN CARDIAC ARREST VIDEO AND VERIFICATION FORM – The student and parent/guardian have watched the OHSAA video on sudden cardiac arrest and both have signed and turned in the verification form to the Athletic Department (video and forms to be released in July 2017).
